nDBForms is a web-based form builder made for Notion. You build a form on top of a Notion database, share it as a link or embed it on a website, and every submission is written to the database as a new page. It targets teams that run hiring, client intake and internal requests in Notion and have outgrown the built-in Notion Forms feature.
Its main additions over Notion Forms are conditional logic on any Notion plan, multi-page forms with a progress bar, Google Drive file uploads, per-form analytics, email notifications, and custom branding with custom domains. The company describes its mission as making data collection as simple as writing a doc, without brittle API integrations.
Setup follows four steps. You sign in with Notion OAuth and choose which pages and databases nDBForms can use. You then pick a database, whose properties become form fields, or start from a template, in which case nDBForms creates the database for you.
In the editor, typing a slash adds a question and dragging reorders them. Conditions, colors and fonts are set there too. Published forms go live as a link, an embed or on your own domain. Responses are written to Notion as soon as they are submitted, with property types mapped, so date pickers become Dates and multi-selects become tags. nDBForms also keeps a copy of each response so failed Notion writes can be retried.
On the form-building side, nDBForms offers conditional logic, multi-page forms with a progress bar and 17 question types, including text, email, phone, URL, number, date, time, select, multi-select, rating, scale and file. Schema change detection re-matches fields when you rename, add or remove Notion columns, so forms keep writing to the right properties.
Uploads of up to 10 MB per file are saved to a Google Drive folder you own, with the link stored on the Notion page. Analytics cover views, starts, completions, drop-off by question, devices, referrers and countries. Email notifications, custom colors, fonts and logo, embeds, custom domains and AI form generation with your own OpenAI or Gemini key round out the set.
nDBForms is a form builder for Notion. You build a form on a Notion database, and each response is saved as a new page in that database. It adds conditional logic, multi-page forms, Google Drive uploads, analytics and custom branding.
nDBForms has a Free plan at $0 with 3 forms and 100 responses a month. Pro is $12/month or $120/year at a founding price, with up to 50 forms and unlimited responses. A Team plan at $49/month is listed as coming Q4 2026.
Yes. Every new account gets 7 days of full Pro access with no credit card. After the trial, the account moves to the Free plan unless you upgrade.
No. nDBForms runs conditional logic in the form itself, so it works with any Notion plan, including Free. On nDBForms itself, logic is a Pro feature after the trial ends.
Uploads are saved to a folder in your own Google Drive, with each file limited to 10 MB. The link is saved on the response's Notion page in a Files property.
nDBForms detects schema changes and re-matches your fields, so the form keeps writing to the right properties. You can rename, add or remove columns in Notion.
Yes. Every form has an embed code for Webflow, WordPress, Framer, Squarespace or any HTML page. Pro also supports serving forms from your own domain.
The company says it only accesses the Notion pages you share during sign-in. It keeps a copy of each response so you can view it in your dashboard and so failed Notion writes can be retried. AI API keys you add are encrypted at rest.
